Following a datix incident it was identified that breastfeeding babies discharged from the neonatal unit were at risk and that these babies frequently did not have appropriate feeding plans in place.
A new form was implemented to notify the infant feeding teams of discharge of a breastfeeding baby and offer direct access to Tier 3 (specialist IBCLC level) support if required.
This has improved care but also highlighted the need for further improvement and collaboration between infant feeding teams and the neonatal unit to improve pre discharge care.
