The General Practice Population Profiles have been produced to assist practices and clusters with their respective development plans, which are a requirement of the Quality and Outcomes Framework GP Cluster Network Development Domain.
These profiles may also be of use to anyone working to improve public health as it has been estimated that 90 per cent of all NHS patient contacts occur in general practice.
This internet release consists of two parts:
(i) The analyses first produced in 2015 and originally made available to practices via the intranet
(ii) An updated and revised chronic conditions analysis
The GP practice population profiles build on the previously published GP Cluster Profiles.
The 2016 updated and revised chronic conditions analysis uses Audit+ data and incorporates feedback received following the release of the original analyses.

Chronic conditions registers 2016 (.xlsx)
Includes: CHD, COPD, Diabetes, Hypertension, Asthma, Epilepsy, Heart failure.
The 2015 analyses include a population based peer grouping exercise, allowing practices to compare themselves with similar practices across Wales. The peer groups were determined following a statistical process which grouped practices depending on their list size, proportion of older people, deprivation and population in rural areas. Further details on how the peer groups were derived can be found in the accompanying technical guide.
The indicators are provided in three interactive data files and cover a wide range of topics from a number of sources including the Census, Welsh Health Survey, Welsh Demographics Service and practice chronic disease registers accessed via Audit+.

To assist clusters with their development plans, the Public Health Wales Observatory has updated and added to the cluster level chronic conditions analyses originally published as part of the GP cluster profiles in 2013.
